Nurse Practitioner Chandler Chastain Joins CHI Memorial Family Practice Associates

CHI Memorial Medical Group is pleased to welcome Chandler Chastain, FNP-BC, to CHI Memorial Family Practice Associates - Cleveland.  Chastain earned a Master of Science in nursing from Vanderbilt University in Nashville. 

Chastain joins Alfredo Molina, MD, and Jamie Bird, FNP-C, MSN, at CHI Memorial Family Practice Associates - Cleveland.  The practice emphasizes disease prevention by focusing on treating the whole person – from childhood to adulthood - with a goal of providing comfortable, convenient, and prompt care to help you get well and stay well.   All providers welcome new patients.
