Ann Collins, DO, Joins CHI Memorial Family Practice Associates - Ooltewah

CHI Memorial Medical Group is pleased to announce Ann Collins, DO, has joined CHI Memorial Family Practice Associates - Ooltewah.  Dr. Collins earned a doctor of osteopathy from Edward Via College of Osteopathic Medicine at Auburn University in Auburn, Alabama.  She completed a family medicine residency at St. Vincent’s East Family Medicine Residency Program in Birmingham, Alabama., and a hospice and palliative care fellowship at the University of Alabama at Birmingham.  She is board-certified by the American Board of Family Medicine.  Dr. Collins also holds master’s and doctorate degrees in human performance from the University of Alabama in Tuscaloosa.

Dr. Collins joins William T. Bates, III, DO, Mark Crago, MD, PhD, Kent Grotefendt, MD. Bruce Johnson, MD, Allen Sherwood, MD, David Winters, DO, Alie Geren, NP-C, Cynthia Shaffer, NP-C, and Joel Smith, NP-C, at CHI Memorial Family Practice Associates - Ooltewah.  

The practice is located at CHI Memorial Ooltewah, 6401 Mountain View Road, Suite 109, Ooltewah, TN 37363.  Clinic hours are Monday through Friday, 7:30 a.m. - 7 p.m., and Saturday and Sunday  8 a.m. - noon.  To schedule an appointment, call (423) 495-5951.  Dr. Collins welcomes new patients.  Same day appointments and video visits are available.
