CHI Memorial and CHI Memorial Medical Group are pleased to provide our patients with access to their health information online using a patient portal. 

MyChart

The MyChart patient portal is available for patients who received services at CHI Memorial Hospital Chattanooga, Hospital Hixson, or Hospital Georgia.

Creating a MyChart Account:

• Visit the MyChart website by visiting mychart.memorial.org
• Select “Sign up Now” 
• To create your account, you’ll need an activation code provided by CHI Memorial. Enter the activation code and other required information. 
• Follow the on-screen instructions to complete the account creating process.
• Once your account is created you can log in to MyChart using your username and password. 

The MyChart app is available in the Apple Store and Google Play.

OneCare / FollowMyHealth

OneCare / Follow My Health patient portal is available for patients who had a clinic visit with CHI Memorial Medical Group physicians or providers. 

Patients who provide an email address during a clinic visit will receive a My OneCare invitation by email. The invitation will be sent from [email protected] and will include instructions on how to register. If you do not receive an invitation check your spam folder. The invitation expires after 60 days.  

1. Creating a OneCare / Follow My Health Account: 
   
2. Click the registration link in the email you receive from [email protected] 
   
3. Create your portal login using a unique username and password.
4. Enter the invitation code provided by your medical provider and click "Agree" to the release of information.

If you already have a portal account, you can add providers to your existing account.  Please call (888) 670-9775 if you have any questions.

The patient portal invitation you receive after visiting one of our facilities will never expire. Once you sign up, the portal will be available to you 24/7.

You can also use another application to access your health information, if you prefer. To do so, you will need to first set up a portal account with My OneCare. The username and password you choose will be the security component for your health information and make it possible for you to access and import it.

The FollowMyHealth app is available in the Apple Store and on Google Play.
